Sensitivity during and after whitening is common, usually transient, and largely manageable. It is not, on its own, a sign of damage - but it is a signal to slow down.
Why peroxide causes sensitivity
Peroxide molecules diffuse through enamel and dentin to reach the pulp, where they can trigger short-lived inflammatory changes. NIH-indexed reviews estimate that between one-third and two-thirds of patients experience some sensitivity during a whitening course.
A four-step reduction protocol
1. Pre-treat with a 5 percent potassium nitrate desensitizing toothpaste for two weeks before starting, used twice daily. Randomized trials show meaningful reduction in whitening-related sensitivity.
2. Choose a lower peroxide concentration. A 10 percent carbamide peroxide take-home gel worn in custom trays produces less reported sensitivity than 35 percent in-office gel while ultimately reaching comparable shade change.
3. Shorten wear time and space sessions. Every other day, or 30 minutes instead of overnight, is a valid protocol when sensitivity appears.
4. Apply a fluoride varnish or high-fluoride gel between sessions if your clinician provides one; this reinforces enamel and reduces cold sensitivity.
When to stop and call your dentist
Sensitivity that lasts more than 48 hours after a session, spontaneous pain, or gum blanching are reasons to pause treatment and be evaluated. These are uncommon with correctly fitted trays and appropriate concentrations, but they are the boundary between expected side effect and a problem that needs professional review.
